Mentor registration form
All mentors are required to complete a Criminal Records
Bureau Disclosure Form before they are matched with any
student(s). If you have been a resident in the UK for less than
6 months, you will not be eligible for a UK police check, and we
will need to carry out a List 99 check instead. For these reasons
we need full details of any previous names or aliases that you have
been known by, and the dates that these names were in use.
Please note:
In order to register as a volunteer Mentor you must be
a UK resident.
